We deliver nutrition-focused, in-home resources – from fresh produce to pantry items to baby essentials – placing health plan priorities right on the kitchen table. Contents are curated per population and accompanied by health-literacy information. Programs can be incentivized for specific HEDIS care gaps and HOS/Stars improvements.
